Health care provider Ascension on Monday shared information about a “privacy incident” affecting patient information across several states, including Alabama, Michigan, Indiana, Tennessee, and Texas.
The incident was discovered Dec. 5 when Ascension learned that patient information may have been involved in a potential security breach.
Ascension’s investigation revealed that the information was inadvertently disclosed to a former business partner, and it was likely stolen due to a vulnerability in third-party software used by that partner. The compromised data includes demographic details such as names, addresses, phone numbers, email addresses, dates of birth, race, gender, and Social Security numbers, as well as clinical information related to inpatient visits.
Ascension is offering two years of complimentary credit monitoring and identity theft protection services through Kroll to those affected. The health care provider has set up a dedicated call center for questions, at (866) 408-3556 on weekdays.
In Indiana, Ascension has facilities in the Indianapolis area, Anderson, Boonville, Evansville, Lafayette, Newburgh, and North Vernon.
