The Hamilton County Health Department invites community members living or working in Hamilton County to participate in a brief survey about their awareness of community resources including health care, mental health services, and food assistance.
The survey is open now and will remain available until 11:59 p.m. Jan. 6, 2027. Participation is confidential and takes only a few minutes to complete.
The survey will measure awareness of local resources and help the Health Department identify ways to make them easier to find as part of the Community Health Improvement Plan (CHIP).

“We have many great resources available in Hamilton County, but we know it’s not always easy to find the right help when you need it,” Health Department Administrator Jason LeMaster said. “This survey will help us understand where we can make that process easier for residents.”
