A Healthier Hamilton County: Systems of Care (HHC:SOC) developed in 2021 from a merger between the formerly grant-funded Hamilton County System of Care (est. 2017) and the non-profit Partnership for A Healthy Hamilton County (est. 2018).
HHC:SOC provides a platform and foundation of partnership to meet unmet needs regarding community wellness throughout Hamilton County based on a consumer-driven approach. The organization remains flexible in its scope of work and can cater to the county’s individual needs.

MISSION – Provide a platform and convening location to foster partnerships that bring people together, lead coordinated efforts that impact and empower residents, facilitate innovative collaboratives that fill in the gaps in service and community education in our community, and strengthen resources for individuals and families that live and work in Hamilton County.
This consortium is a collaborative space that fosters partnerships that bring people together with a purpose, driving forward innovative initiatives that address service and community education gaps, while bolstering the sharing of resources. Members are diverse and inclusive of any service provider that provides for Hamilton County’s residents.
HHC:SOC meets once a month to collaborate, learn about new programs in the county, and discover how members each support the community and where they connect. These conversations in this space identify the gaps in communities – what is being missed and what could enrich Hamilton County even further.
HHC:SOC is flexible enough to adjust and develop initiatives that meet new needs. The organization leads the Youth & Family Alliance, which has adjusted its projects over the years. In 2025, the community need is to bolster youth voices in the Alliance, create a project or program from those voices, connect with local partners to build the project, then present to the community. The Youth & Family Alliance is open to all young people, parents, grandparents, and families with young people in their lives.
The Stronger Veterans, Stronger Communities (SCVC) Coalition on the Prevention of Suicide in Service Members, Veterans, and their Families in Hamilton County is also led by HHC:SOC. Local leaders identified that veteran suicides were increasing and recognized that a critical gap in veteran suicide prevention existed, and A Healthier Hamilton County was asked to leverage its established networks to build and expand a responsive coalition and strengthen resource connections. The organization continues to develop ways to identify county veterans and their families, connect them with the resources and services they need, create a sense of purpose and belonging, and advance suicide prevention and safety channels for those in a suicidal crisis.
HHC:SOC has also focused on initiatives in early childhood ‘Making Care Choices for your Family: A Process for Systems of Support), presenting on Adverse Childhood Experiences and Positive Childhood Experiences & Resiliency, Question Persuade Refer Suicide Prevention, sharing of resources, and more. Trainings are available on all topics mentioned here through HHC:SOC. HHC:SOC also continues to have the capacity to refine and implement efforts, initiatives, and trainings responsive to community interests.
