All immunization records or exemptions must be received by your school nurse by Aug. 8, 2024. This is a requirement per state law for every student enrolled in Indiana schools.

If you are new to the district – or returning and received a message from the school nurse about missing immunization information – please continue reading. (If your student is up to date, and you did not receive a message from the school nurse, no action is required.)
Note: all religious or medical exemptions expired the last day of school on May 30. A new exemption form must be signed and received by the start of each school year to be considered up to date. Click here to access forms.
To meet the immunization deadline, plan ahead, as appointments fill up quickly over the summer.
Immunizations can be given at:
- Hamilton County Health Department
- Your child’s healthcare providers office
- Local minute clinics (i.e., Kroger, CVS, etc.)
- Trinity Free Clinic
- Heart and Soul Clinic
