As part of the Fishers Health Department’s ongoing quality improvement efforts, the department are asking for feedback on its response to the COVID-19 pandemic via an online survey.
The survey will take five minutes or less for most people to complete. The survey gives you the opportunity to provide feedback on:
- What did FHD do well?
- What can FHD improve?
- Review of the services you used
- Value of services provided
- and more.
This survey will inform Fishers city personnel, public health, and public safety staff as part of its after-action report to inform future crisis responses.
