Crash on Keystone Avenue results in one fatality

At 6:47 p.m. on Wednesday, Aug. 15, Carmel Police responded to an accident on northbound Keystone Parkway just south of 136th Street. Officers arrived and found two vehicles involved in the crash.

Based on the preliminary investigation, it appears a beige Toyota Camry was traveling southbound Keystone Parkway near 136th Street. For some unknown reason, the Toyota crossed the median south of 136th Street into the path of a white Ford F-150 truck traveling northbound on Keystone Parkway.

The driver of the Toyota, Youssef S. Youssef, 58, Carmel, was taken to St. Vincent 86th Street where he was later pronounced dead as a result of the accident.

The driver of the Ford F-150, Brian Zawadzki, 39, Whitestown, only had minor complaint of pain. Zawadzki had three passengers in his vehicle, all children ages 6, 11 and 15. All the children were taken to Carmel St. Vincent hospital where they were later released with complaint of pain.

Northbound Keystone Parkway at Main Street was closed for several hours while the accident reconstructionists from Carmel Police Department investigated the crash.
