The Hamilton County Sheriff’s Office is investigating a single-vehicle fatal crash that occurred at approximately 2:39 a.m. on Thursday, Aug. 2.
Deputies were dispatched to the area of Middletown Avenue and Cyntheanne Road in the eastern part of the county on the report of a crash with an unresponsive individual involved. Along with the driver trapped, a passenger was partially trapped inside the vehicle.
Deputies responding to the scene located the crash on Middletown Avenue approximately 2,500 feet west of the intersection with Cyntheanne Road. The initial crash report indicates a tan 1990 Cadillac Deville driven by Shawn Butler, 38, Cicero, was westbound on Middletown Avenue when it left the north side of the roadway. Investigators believe Butler over-corrected causing the car to cross the roadway and run off the south side of the pavement before striking a utility pole.
Butler was pronounced dead from multiple injuries. Rachel Butler, 33, Cicero, a passenger in the vehicle, was transported to St. Vincent Hospital on 86th Street for a possible head injury. She is the wife of the deceased.
Per policy, toxicology tests were performed to determine if alcohol or drugs were involved in the crash but it will be some time before results are received.
The crash is under review by the Hamilton County Crash Investigation Team. Anyone with information can contact the team at 317-773-1282.
